How do I redeem my SoClean 2 $50 manufacturer rebate?
After purchasing a SoClean2 from Hometown Medical, you can redeem your $50 manufacturer rebate online or by mail. Rebate is independently offered and provided by the product manufacturer and not Apria Healthcare LLC or its divisions or affiliates, including ApriaDirect.
Mail – To redeem your rebate by mail:
- Download and print the SoClean Mail-In Rebate Form
- Fill out and enclose the completed mail-in rebate form.
- Enclose your dated proof of purchase in the form of an order confirmation, receipt, or invoice for a SoClean 2.
- Mail all items in a single envelope by August 30, 2020 to:

Limit 2 rebates per household. Offer valid for purchases dated 3/2/2020-06/30/2020. Rebate amount is $50*.
Redemption must be submitted online or by mail within 60 days of purchase.
